Abstract
Background: Health system responsiveness is a non-clinical quality of care that
reflects the expectation of clients on service delivery. However, there have been a few
evidences on the health system responsiveness for under-five children cares in Ethiopia
Objective: Theaim of the study was to assess health system responsiveness for underfive
children health services at primary healthcare units in Dabat district, Northwest
Ethiopia.
Methods: An institutional-based quantitative and qualitative triangulated mixed study
design was carried out between May,27/2022to June, 22/2022.A systematic random
sampling technique was employed on 624 participants and data were collected using
interviewer administrated structured and pretested questionnaires. Both bivariable and
multivariable logistic models were used to identify factors associated with health system
responsiveness. Adjusted Odds Ratio(AOR) with their corresponding 95% CI was used
to declare factors associated with health system responsiveness. A p-value less than
0.05 were used to declare significant statistical variables.On the other hand, five key
informants and five in-depth interviews were done for the qualitative study. The KI and
ID interview processes were tape recorded and transcribed. Finally, the recorded data
was translated and thematic analysis was conducted using open-ended software.
Results:The overall magnitude of good health system responsiveness was 76.1% with
(95%; CI: 72.6-79.3).The highest and lowest scoring domains were confidectiality
(84.0%) and choice (37.7%).Having formal education status (AOR = 2;95% CI(1.03-
4.60), service in health post (AOR = 0.2;95% CI (0.10-0.40) and client satisfaction
(AOR= 5.6;95% CI(3.50-8.90)were factors that significantly associated with the HSR of
the under-five children health service. Data from key informants and in-depth interviews
(IDIs) support the responsiveness of the health system for children under the age of five
that described in quantitative analysis
